LPN (Licensed Practical Nurse)

*Core Values* Teamwork - Integrity - Excellence - Compassion-Accountability Responsibilities: - Provide direct patient care, including administering medications, wound care, and monitoring vital signs - Assist with physical examinations and diagnostic evaluations - Collect and process specimens for laboratory testing - Collaborate with healthcare team members to develop and implement patient care plans - Provide basic life support and emergency medical care as needed - Maintain accurate and up-to-date patient records and documentation - Ensure compliance with HIPAA regulations and maintain patient confidentiality - Assist with discharge planning and coordination of post-hospital care - Provide case management services to patients and their families - Administer medications via various routes, including oral, intravenous, and intramuscular Qualifications: - Valid LPN license in the state of West Virginia - 1 year of Home Health Care experience preferred but not required Note: This job description is not intended to be all-inclusive.
